Address the Social Determinants of Health
SDOH are the personal, social, economic, and environmental conditions in which people are born, grow, live, work and age. These determinants are influenced by the unequal distribution of power, money and resources at local, national and international levels.
Therefore, they have a significant impact on people's access to healthcare and their overall quality of life.
Sociants has adopted 13 social determinants of health, inspired by the conceptual framework developed by the Centers for Medicare and Medicaid Services (CMS).
